*Does not include jewelry price* A parent or legal guardian MUST be present at the time of the service. The parent or guardian must present their State or Federally-issued Photo Identification as well as a photocopy of the minor client's birth certificate, and a form of Photo Identification for the minor (School ID card, Passport, Library card with a Photo, and/or a school or yearbook photo that includes the minor's name with the photo). For legal guardians and/or emancipated minor, we must have court paperwork proving so (legal, notarized document from the courts). ** Please Note- A FLDOH/Martin County Department of Health Minor Consent Form, must be completed and signed by a Notary Public.** PLEASE REFER TO THE WEBSITE PAGE "MINORS" FOR MORE INFO
